Pitted scars are not a surface pigment problem, but structural collapse in the dermis — they need a precise repair strategy.

When lasers, microneedling and peels have all been tried and acne scars still have not budged, what you need is not more treatments, but a more precise repair strategy.
In Hong Kong, atrophic acne scars (pitted scars) are among the most underestimated skin problems. Many people assume they are simply “slightly deeper post-acne marks”, and try countless brightening serums, spot-fading creams, or even multiple laser sessions, only to find those pits still standing their ground — because pitted scars are not a surface pigment problem at all. They are structural collapse formed after collagen and elastic fibres in the dermis have been destroyed.
Our clinic exists precisely for this.
Why are pitted scars so difficult? — First understand the three forms of “the pit”
Pitted scars are not a single type. They must be treated according to subtype; otherwise, using the wrong method will only make things worse:
| Type | Features | Common misjudgement |
|---|---|---|
| Ice pick | Deep and narrow, like a hole pierced by an ice pick, diameter less than 2 mm | Often mistaken for enlarged pores; ordinary cleansing products cannot improve them at all |
| Boxcar | Round or oval depressions with sharp edges and a flat base, diameter 1.5–4 mm | Often mistaken for ageing laxity; filler-style skincare is completely ineffective |
| Rolling | A wave-like undulating surface with indistinct edges, caused by fibrous tethering in the dermis | Often mistaken for puffiness or fat accumulation; wrongly using fat-reduction treatments can make them worse |
We insist on diagnosis first, treatment second, because using an ice-pick approach on rolling scars not only wastes money, but may also delay the real window for repair.
Our treatment strategy — not “one device for everything”, but “composite, stepped reconstruction”
Unlike the typical clinic approach of “one laser for all”, we flexibly combine the following techniques according to scar depth, width, edge morphology and skin thickness, repairing layer by layer:
Layer one: deep release (where indicated)
For rolling scars and deeper boxcar scars, we first use subcision to release the dermal fibrous bands tethering the epidermis, allowing the depressed area to return towards a flatter plane and creating space for subsequent reconstruction.
Layer two: energy stimulation
The most suitable device is chosen according to scar type:
· Fractional carbon dioxide laser: for ice-pick and boxcar scars, it vaporises scar tissue and stimulates abundant new collagen — the gold standard for treating deep atrophic scars.
(Depending on the individual case, we may also flexibly combine other adjunctive energy modes, but the core repair remains centred on fractional CO₂ laser, to ensure precision and results.)
Layer three: personalised aftercare and a long-term maintenance plan
Half of the success in treating pitted scars comes from the device; the other half comes from aftercare management. We therefore place particular emphasis on the following:
· Treatment frequency is individual: we decide the interval between sessions according to your skin thickness, repair capacity, scar severity and response to energy. In general we recommend an assessment every 4 to 6 months, rather than a rigid fixed number of sessions. Those with better skin repair capacity may need only one maintenance treatment a year; those with more fragile skin need longer for collagen to regenerate stably.
· Home aftercare guidance: the first month after treatment is the golden period for collagen remodelling. According to your skin condition, we prescribe dedicated repair products, including but not limited to growth-factor serums, high-performance hydrating repair creams and gentle antioxidants, and give detailed guidance on application order, frequency and amount, so that newly formed collagen can align properly rather than proliferate in a disordered way.
· Regular follow-up and adjustment: we do not consider the process finished once a course is done. We arrange regular reviews to observe repair progress and, if needed, adjust the home-care plan, so that you reach the best possible repair in the shortest time.
How we differ — why we can do what others cannot
· Not a “package treatment”: we do not have one “acne-scar package” for everyone. Everyone’s scar combination is different, and our plan is tailored.
· We accept referred cases: we frequently see clients who have had multiple laser sessions at other clinics with no improvement, or even worsening. We do not comment on previous practitioners, but we are willing to take on difficult cases.
· Honest about what is not feasible: if the scarring is too severe, we will not proceed just to earn a treatment fee. We will tell you honestly, and suggest other viable directions.
Our ultimate promise
“If even we cannot improve it, we will tell you directly — and then help you look for other possible paths.”
Because we believe every treatment should be built on honesty and professionalism, not on sales targets.

📍 Who it is for
· Those who have had multiple lasers, microneedling or peels, with no clear improvement in pitted scars
· Pitted scars accompanied by severe rebound darkening or uneven pigment
· Two or more different forms of atrophic scar present on the face at the same time
· Those looking for a clinic that is truly “pathology-led” rather than “sales-led”
If you or someone close to you has been troubled by pitted scars for years, you are welcome to come for a consultation with previous treatment records. We will first carry out a detailed scar-type assessment, then provide reasonable expectations and practical advice.
Because every scar deserves to be taken seriously.
